Chronotropism in CPET - Is incompetence limiting functional capacity?

Introduction Chronotropic incompetence (CI) is defined as the inability to reach 80% expected reserve frequency for age during exertion, and it frequently observed in stress tests patients undergoing cardiac rehabilitation programs due a combination factors, that include use betablockers. Despite theoretical basis suggesting lower peak heart rate related tolerance we lack data correlating CI maximal functional capacity, measured cardiopulmonary oxygen volume consumption (peak VO2). Purpose To correlate impact on capacity (pts) program try find best value chronotropic could predict worse prognosis. Methods Prospective cohort study which included consecutive pts who were submitted exercise test (CPET) 5 years tertiary hospital. Demographic analyzed medication with beta blocker was registered. index calculated using equation (220-age) estimating maximum HR. ROC curve method Kaplan-Meier survival analysis used evaluate cut-off efficacy. Results We 358 CPET 74,1% (n=206) under therapy (7,6% high dose). The majority had (83.5%); more frequent BB although didn’t statistical significance (p=0.12). A low predicted (mpHR) associated VO2 (p=0.02) mpHR < 61% VO2< 12mL/(kg.min) (AUC=0.746, S=72; E=65), despite not showing association cardiovascular events. Regarding chronotropism, 96.9% categorized having . 0.38 VO2<12 mL/(kg.min) (AUC=0.81, S=72, E 76) predictor all-cause hospitalizations (p=0.015). Conclusion This shows percentage CI. cronotropic associatated hospitalizations.
